The Role of Carnitine in Cellular Energy Production
In the body, carnitine can be synthesised, primarily in the liver and kidneys, from the essential amino acids lysine and methionine. Under certain conditions the demand for carnitine exceeds an individual’s capacity to synthesise it, making it a conditionally essential nutrient.1,3

A large number of cofactors are required for the synthesis of carnitine in the body including S-adenosyl methionine (SAMe), magnesium, iron, α-ketoglutarate and vitamins C, B6, B2 and B3.1

Although found in almost every cell of the body, carnitine is concentrated in the tissues that use fatty acids as their primary fuel source, such as skeletal and cardiac muscle.3

In these tissues, short- and medium-chain fatty acids, due to their small size, are able to cross into the mitochondrial membrane via passive diffusion.4 However, long-chain fatty acids require carnitine to transport them onto the mitrchondria before they can be processed by beta-oxidation to produce cellular energy in the form of adenosine triphosphate (ATP).1,3,4

Following the delivery of long-chain fatty acids into the mitochondria, carnitine is free to return to the cytosol to be used again, or to accept short- and medium-chain fatty acids that have been transferred from coenzyme A in the mitichondria. Carnitine then transports these fatty acids out of the mitochondria into the cytosol. In this way, carnitine prevents the mitochondrial accumulation of short- and medium-chain fatty acids, and helps to restore the mitochondrial pool of free
coenzyme A, a cofactor require for numerous cellular reactions.3
